INSURANCE CARD INFORMATION (APRIL 5TH, 2010)

Starting July 1, 2010, If you do not present proof of insurance, are a self pay patient, or have a deductible that has not been met, you will be required to pay $50.00 at the time of check in that will be applied to that days visit.

In an effort to accommodate all of our patient's who need to be seen, if you do not cancel your well child appointment within 24 hours, or a sick visit within 2 hours, you will be charged a $25.00 no show fee.
