Medical Home

A medical home is a place where you can find refuge and relief regarding your medical needs, concerns and questions. It is not a building of walls, but a team-based health care delivery system set up for your child. A medical home can be your medical family.

We will assist you in the coordination of care with other health specialists, home health agencies, support groups, and community resources. We will provide educational opportunities that will help you to have a better understanding of your child's health care needs. 

Through a medical home coordinator and our caring staff, we strive to advocate for your child and provide high quality care and chronic condition management across a lifespan. We know health care systems can be complicated and we want to help you and your family navigate for needed care.

The American Academy of Pediatrics (AAP) describes the ideal Medical Home as one that provides "accessible, continuous, comprehensive, family centered, coordinated, compassionate, and culturally effective care". Though many Medical Home implementations focus on children with special needs, every child deserves a Medical Home.
